Most proceedings begin with a Form C1 to the Family Proceedings Court (FPC).  For information about how to apply, click here.

If you believe the child has suffered or is at risk of suffering any harm through domestic abuse, violence,  other conduct of behavior, or ill treatment of others, you will need to fill out a Form C1AA and submit it with a Form C1.  This is the form necessary if you answer "yes" to Question 6A in the Form C1

  The Family Care Centre and High Court have a different Form C1. 

C2 Application: for leave to commence proceedings; for an order or direction in existing proceedings; to be joined as, or cease to be, a party in existing family proceedings.

A judge may ask you to write a Position Statement detailing your views and thoughts on the Application before the Court in relation to the child.  Generally, a position statement is relatively brief and would explain the backround of the issues and your desired outcome.

An Affidavit is a sworn statement.  It will need to be notarized.

If the Judge calls for a Witness Statement or Statement of Evidence, you may use this template to help with formatting and structure.

If you seek enforcement of an Order from the Court, you begin the process by filing a Contempt Summons with the approrpriate Court Office. 

Article 7(1)(b) and (2) of the Children (Northern Ireland) Order 1995 allows for Parental Responsibility Agreements.

This form is used for a notice of motion to bring enforcement proceedings in the Family Care Centre.  This begins the process by which you would make an application try to get the Court enforce and existing order issued by the Family Care Centre.
